The language section covers grammar, reading comprehension, and vocabulary and general knowledge. Registering for testing at a college or university requires calling the school to schedule or register online and creating an Evolve account. Test fees may vary by institution. What’s on the HESI A2?

The HESI A2 test is actually split into eight subtests, each covering a different subject. Depending on where you take the test, the time limit and number of questions in each subtest may be a little different, but there are generally around 300 questions in total across the full test.

Here’s the good news: you likely won’t be asked to take all eight subtests .

There is no general passing score for the HESI. 

Each institution sets its standard for scoring. Some require a minimum of 75% as a cumulative score, while others require between 80-90%. 

Some schools will have set scoring requirements for certain sections, such as a minimum of 75% in Mathematics or 85% in Reading Comprehension. 

How many times can I take the HESI?

The number of times you can take the HESI depends on the institution you are applying to. There are two types of limitations set by colleges/universities: 

Number of attempts per year: Some schools might only allow a certain number of attempts per set period of time. For example, you are allowed to take the HESI twice during a 12-month period, and you can try again after the 12 months have passed. 

Total number of attempts: Some institutions allow students a limited number of attempts in total, usually around 2 or 3. This means that you will not be allowed to take the HESI again at that institution after your second/third attempt. 

How much does the HESI cost?

The cost of the HESI exam can vary, depending on how and where you choose to take it. Expect to pay anywhere between $30 to $110. 

On average, many schools set their HESI fee at about $45 , with an additional $25 proctoring fee if you take the test remotely.  

Some schools will allow your first attempt to be free, meaning you will only have to pay if you need/choose to retake the HESI. 

Check your school's policies regarding costs and payment, as they will have the most updated and relevant information for their HESI exam sessions. 

What happens if I miss my HESI exam because of an emergency? 

If you miss your HESI because of an emergency, you will most likely be allowed to reschedule. 

Expect to be asked to submit proof of the emergency since many schools will not allow you to reschedule under other circumstances.

Where Can I Take the HESI A2 Nursing Exam?

Two options exist for students who want to take the in-person HESI exam. 

You can take the exam at the college/university you want to apply to. Usually, the institutions that require the HESI as an entrance exam will hold exam sessions on-site.

You can also take the exam at a Prometric testing center. Before registering, research such centers and their availability in your area. 

When should I schedule the HESI?

You should schedule your HESI exam at least 72 hours in advance. 

However, always check with your school to see how they handle the scheduling process , as they might have different guidelines and requirements.

You can register for the HESI A2 through the Prometric website or your institution. 

If you are taking the test through Prometric, either in-person or online, you can access the Prometric website. From there, you select the 'Schedule' option from the left-hand panel on your screen.

You can also call 1-800-481-6457, Monday through Friday, from 8 a.m. to 9 p.m. (ET).

If you are taking the exam at an institution, you should check their HESI registration instructions. Some will ask you to register through Evolve, while others will have their own systems in place.

How many questions do I need to get right to pass the HESI?

There is no set number of questions you need to get right to pass. 

The questions are worth different points, depending on their difficulty, so the result you receive is not determined by how many answers you got right. 

What is a Good HESI Score?

According to Evolve, a good HESI score is a minimum of 850 . This score will be converted into a percentage of around 80%, depending on which questions you missed. 

How long do HESI scores last?

On average, HESI scores are good for about two years. 

However, this can vary from institution to institution . Some will allow HESI scores that are older than two years, while others only allow scores received in the same year as the admission window. 

When should I start studying for the HESI A2?

You should start studying for the HESI at least 6 weeks in advance. This will ensure that you can cover all the material thoroughly. 

However, each person studies at their own pace. Assess your needs and decide how much time you require to have a satisfactory understanding of all the modules. 

What do I need to study for the HESI?

For the HESI, the subjects you need to study depend on the modules required by your program. The most tested modules are Mathematics, Reading, Grammar, Biology, Chemistry, and Anatomy & Physiology.

You need to bring an approved form of identification: a driver's license, passport, or military ID. 

Some exam centers ask you to have the receipt of your paid testing fee.  

You can also bring drinks or snacks. You cannot bring them into the examination room, but you can leave them in lockers or other designated spaces. Usually, you are allowed to access these items during break time. 

Can I wear a head covering during the HESI?

You can only wear religious head coverings during the HESI A2. 

Any other type of head covering, such as hats, caps, or hoods, is not allowed. 

How can I apply for HESI A2 accommodations? 

To apply for HESI A2 accommodations, you will need to submit a form/request.

If you are taking the HESI at a Prometric exam center, you can submit a request through the Evolve/Elsevier registration portal. 

If you are taking the exam at an institution, they will have an outline of their accommodation submission process on their website. Usually, you are required to submit a form at least 30 days before your exam date.

HESI A2 Outline

The HESI A2 exam contains seven subtests, which are outlined below.

It’s important to note that you will most likely not be required to take every subtest on this list.

Depending on your program, you may only need to take the Mathematics, Reading Comprehension, Vocabulary, and Grammar subtests. Some schools may ask you to take the Biology and Anatomy & Physiology subtests as well, but the Chemistry subtest is rarely ever required. Check with the administrators of the school you are applying to for more information.

How to study effectively.

Your success on HESI test day depends not only on how many hours you put into preparing, but also on whether you prepared the right way. It’s good to check along the way to see whether your studying is paying off. One of the most effective ways to do this is by taking HESI A2 practice tests to evaluate your progress. Practice tests are useful because they show exactly where you need to improve. Every time you take a free HESI practice test, pay special attention to these three groups of questions:

• The questions you got wrong
• The ones you had to guess on, even if you guessed right
• The ones you found difficult or slow to work through

This will show you exactly what your weak areas are, and where you need to devote more study time. Ask yourself why each of these questions gave you trouble. Was it because you didn’t understand the material? Was it because you didn’t remember the vocabulary? Do you need more repetitions on this type of question to build speed and confidence? Dig into those questions and figure out how you can strengthen your weak areas as you go back to review the material.

Answer Explanations

Additionally, many HESI practice tests have a section explaining the answer choices. It can be tempting to read the explanation and think that you now have a good understanding of the concept. However, an explanation likely only covers part of the question’s broader context. Even if the explanation makes sense, go back and investigate every concept related to the question until you’re positive you have a thorough understanding.

Comprehend Each Topic

As you go along, keep in mind that the HESI A2 practice test is just that: practice. Memorizing these questions and answers will not be very helpful on the actual test because it is unlikely to have any of the same exact questions. If you only know the right answers to the sample questions, you won’t be prepared for the real thing. Study the concepts until you understand them fully, and then you’ll be able to answer any question that shows up on the test.

You may have heard this test called “the HESI.” But it has quite a few aliases. Other names for this notorious exam include:

• HESI Evolve
• Evolve Reach HESI
• Evolve Reach Admission Assessment 

The names all refer to the HESI A2 test for prospective nursing students, which gauges their academic skills. It evaluates students’ readiness for  successfully completing the demanding nursing curriculum . 

The HESI A2 Exam is computer-based and includes multiple-choice questions. It consists of subject-specific sections. There are eight academic subjects and three areas of aptitude. Some schools require applicants to take four subjects, and some require five or more. You can see examples of the HESI A2 subject tests here , to see where you stand in each area. Test-takers must schedule the subjects required by the school they want to attend by working with a college advisor. 

Many schools offer HESI A2 testing on-campus. Others allow students to test off-campus at approved test centers. The exam is administered through Prometric. 

Prometric offers in-person and virtual test appointments. Depending on the school’s requirements, some candidates are eligible to test remotely from anywhere with an internet connection by way of a remotely proctored exam.

Academic Subjects

Anatomy & Physiology: The HESI Anatomy & Physiology (A&P) test evaluates students understanding of body parts and functions. The topics covered in this test section include basic terminology, fundamental anatomy and body systems, anatomical planes and directions, and histology. Biology :  The HESI Biology test examines the test-takers knowledge of plants, animals, and microorganisms. Test questions cover general biology basics, cellular respiration, photosynthesis, metabolism, biological molecules, cells, genetics, and DNA. Nursing schools with Biology courses as prerequisites may require students to take this HESI A2 section. Chemistry :  The HESI Chemistry section tests students on chemistry concepts. Mathematics : The HESI A2 Mathematics exam assesses students’ understanding of mathematical concepts, specifically those used in healthcare. The test includes questions about basic operations, decimals, ratios, fractions, military time, algebraic equations, roman numerals, measurements, and conversions. Aptitude Assessments

In addition to the academic subject tests, the HESI A2 Exam also has three sections related to student aptitude. These include:

• Learning Style (14 total questions, 15-minute time limit)
• Personality Style (15 total questions, 15-minute time limit)
• Critical Thinking (A2CT) Test (30 total questions, 30-minute time limit)

Most nursing schools require math, reading, vocabulary, and grammar. Some require biology, A&P, and A2CT. Since each school determines which sections students must take and the minimum required scores, check with your school before you start studying to pass the HESI A2. 

How to pass the HESI A2 Exam?

Each subject exam is scored out of 100. Final subject scores may also be grouped into a combined score that your school uses to rank you against other students. Your school will determine what a passing score is. But most programs consider 75% to be the passing grade. Be sure to check with your school if you have any questions about what it considers a passing score.

Test results are sent to your school when you finish the exam. But don’t worry. You won’t die from anticipation. Scores are also available immediately when you finish the computerized test. And HESI A2 Exam results are loaded to your Evolve Elsevier web account within 24 hours after the test. 

Although schools determine the passing scores, here is a good way to look at your results.

• 90-100% Excellent score. You rocked it!
• 80-89% Very good. Strong score.
• 75-79% Good score, satisfactory to pass
• Less than 74% Poor score. Keep working! 

Candidates who do not achieve passing results may have to retake the test. That could mean a waiting period. Students should check with their school program to find out their policy for retaking the HESI A2 test.

How do i schedule an exam.

Contact your school to find out your options. Some schools only offer the exam online through Proctor U. Other schools have testing centers that allow in person testing. Your best option is to reach out the program or school you’ve chosen and ask them about your options.

How much time is allotted for the exam?

The amount of time you receive depends on your school. We have seen anywhere from 3-6 hours depending on the school and what sections they require you to take. Contact your school to learn more about how much time you have and what will be on your exam. 

What should I study?

Is scratch paper allowed?

Yes, generally you are given 2-3 pieces of scratch paper. At home testing also allows you to have scratch paper. Make sure to show your online proctor that you have scrap paper and how much scrap paper that you have. 

How many breaks are given and how long are they? And do we ask or are they given automatically after each section?

You can take a 5-minute break between each section of the test. The testing center will check your photo ID before you are allowed to reenter the exam room and start the next test section.

Can we drink something while taking the test?

If you are in a testing center usually the answer is no. It might be different if you are taking the test online so make sure to ask your proctor. 

Can we go back to previous questions?

No, you cannot skip questions and go back to them. If you skip a question, it will be considered incorrect.

What else should I bring apart from my ID?

You only need your ID. They will tell you if you need anything else. Reach out to your school if you have any questions about things you need to bring or want to bring. 

I know it is proctored so will someone be on screen while I take the test? Like Zoom?

They are able to see you but you cannot see them. 

Who do I contact if issues arise during my exam?

If you are in a testing center, talk to the proctor there. If you are online contact the proctor site immediately. 

Can I skip sections?

Usually, the school will only have the sections they require show up on the test. Others have skipped sections that they did not need to take, and the school did not count it against them.

How long do I have to wait to retake the exam?

You can take the HESI exam only once every 60 days but no more than 3 times in a 12-month period.

How many times can I take the exam?

This depends on your school. Some schools allow more than one retake while others only allow one attempt. 

Can I reschedule the exam if I am not feeling prepared to take it?

In order to reschedule your exam, you need to cancel your original test date 24 hours before you are scheduled to take it. Once you do that you can sign up and pay for a new test. 

How does the HESI compare to the TEAS?

The HESI A2 exam is generally considered easier than the TEAS. The HESI is broken up into these 8 subjects: physics, biology, chemistry, anatomy & physiology, grammar, reading, vocabulary, and math. The TEAS test only covers math, reading, science, and english. 

If someone is taking the test from home, can we wear earplugs or headphones?

In most schools, you are not allowed to unless you have an accommodation. Make sure to reach out to your school in order to find out their specific policies.

HESI A2 with Critical Thinking

Before taking this test I searched everywhere to try and find out what the critical thinking section would be like. I really couldn't find anything useful in easing my unease about that part of the test, so I thought I might post something to help others who take the HESI with the CT section.

The information I received with my CT scores follows:

This exam is a four-choice, multiple-choice test. All answers are correct. However, each choice reflects a different degree of critical thinking skills.

Scores range from 0-1000. The higher your score is, the better your ability to think critically within the discipline of nursing.

In addition to a total score on this exam you will receive scores on five sub-categories of critical thinking:

Problem Solving: These questions refer to the process of inquiry in which the nurse seeks multiple facts to remove obstacles or resolve patient care problems.

Biases and Ethical Dilemmas: These questions address ethical, legal, and moral issues, as well as biases nurses may have about patients and others.

Argument Analysis: These questions refer to situations in which the nurse seeks to resolve conflicts or disagreements by considering multiple points of view.

Analysis of Data: These questions address the need to interpret patient data correctly, and decide if further nursing action is required.

It says five sub-categories, but it only details four. Prioritization of patient care is not listed, but I think the title is very descriptive. Someone on another post suggested a practical nursing book, but I didn't have a chance to find one and study it. I scored 870 overall on the CT section. Can a nurse accept muffins as a thank you? I know that I myself would want to say thank you with baked goods, but I don't know if that's something that should be done. I also am not certain to what extent an off duty nurse should diagnose a stranger. It's obvious from my scores that I needed more knowledge regarding Analysis of Data and Argument Analysis.

Hi. Thanks for posting this :)How many questions is the critical thinking section only? I read that's it is 25 questions... Was that true when you took it?If every choice is correct, do you get partial points for answers that would have been the "next best answer"? (hope that makes sense)I'm studying out of the Fundamentals book... For those who studied out of that book, are there sections I should focus on more?Do I need to know lab ranges?Thank you in advance...

I completed the hesi not sure where the critical score is i saw I made an 800 but now showing on my paper profile page is this separate . I dont see it listed only the personality ect which was listed last please help and ty :)

wow that was on my test too. i said direct to pharamacy about the diagnosing and share the muffins with the rest of the staff lol but im not sure. i received an 840 on that section

no i didn't use any books. the questions they ask you have to basically think if you was in that situation as a nurse and choose the best solution or action to take. it was kind of easy if you know the guidelines and do's and dont's of being a nurse.

I will be taking the Hesi exam next week. I just a had a quick question of any of y'all happen to know. Since all answers are correct and places you in a category of CT does this help or affect your final score? Thank you

Wow, I just took the CT exam, and I'm surprised that the questions (5 years after the original post) seem almost the same. I took the "muffins" (a box of chocolates on my exam) from a patients son, and shared them with the staff because it was a nice gesture, and to refuse seemed offensive. Regarding the question in the pharmacy (while off-duty), I chose to refuse to offer medical advice because I'm not a doctor. I also got the question about the patient defecating on themselves in the bed (chose to clean the patient first, and use towels underneath him until I could find clean sheets).

Honestly, there is no magical way to approach the questions. I just really tried to imagine myself in the situation, and decide what action I would take. For example, if I need to change bed linens for a patient, and the floor was out of linens, I would take them off of an unused bed. If I was in the middle of changing a dressing for a home health patient, and another patient called, I'd silence the call until I was done; however, if I was at dinner with friends, and I was on-call, I'd leave the table to take a call. If a patient just got a cast on their lower leg, and it became very painful, the first thing I'd check is their their toes to make sure there is adequate circulation. Really, all you need to do is think about what you'd do as a professional, and perhaps also think what you'd like a nurse to do for you if you were the patient. However, for the question about the guy who wanted to meet the nurse for coffee to discuss the physician, I chose to "explain professional boundries"--not everything the patient wants is acceptable :-). Overall, I scored 940, not the highest, but good enough. Good luck to all future HESI testers!
