Everyone has their own way to “recharge” their sense of well-being — something that makes them feel good physically, emotionally, and spiritually even if they aren’t consciously aware of it. Personally, I know that few things can improve my day as quickly as a walk around the block or even just getting up from my desk and doing some push-ups. A hike through the woods is ideal when I can make it happen. But that’s me. It’s not simply that I enjoy these activities but also that they literally make me feel better and clear my mind.

Mental health and physical health are closely connected. No kidding — what’s good for the body is often good for the mind. Knowing what you can do physically that has this effect for you will change your day and your life.

Physical activity has many well-established mental health benefits. These are published in the Physical Activity Guidelines for Americans and include improved brain health and cognitive function (the ability to think, if you will), a reduced risk of anxiety and depression, and improved sleep and overall quality of life. Although not a cure-all, increasing physical activity directly contributes to improved mental health and better overall health and well-being.

Learning how to routinely manage stress and getting screened for depression are simply good prevention practices. Awareness is especially critical at this time of year when disruptions to healthy habits and choices can be more likely and more jarring. Shorter days and colder temperatures have a way of interrupting routines — as do the holidays, with both their joys and their stresses. When the plentiful sunshine and clear skies of temperate months give way to unpredictable weather, less daylight, and festive gatherings, it may happen unconsciously or seem natural to be distracted from being as physically active. However, that tendency is precisely why it’s so important that we are ever more mindful of our physical and emotional health — and how we can maintain both — during this time of year.

Roughly half of all people in the United States will be diagnosed with a mental health disorder at some point in their lifetime, with anxiety and anxiety disorders being the most common. Major depression, another of the most common mental health disorders, is also a leading cause of disability for middle-aged adults. Compounding all of this, mental health disorders like depression and anxiety can affect people’s ability to take part in health-promoting behaviors, including physical activity. In addition, physical health problems can contribute to mental health problems and make it harder for people to get treatment for mental health disorders.

The COVID-19 pandemic has brought the need to take care of our physical and emotional health to light even more so these past 2 years. Recently, the U.S. Surgeon General highlighted how the pandemic has exacerbated the mental health crisis in youth .

The good news is that even small amounts of physical activity can immediately reduce symptoms of anxiety in adults and older adults. Depression has also shown to be responsive to physical activity. Research suggests that increased physical activity, of any kind, can improve depression symptoms experienced by people across the lifespan. Engaging in regular physical activity has also been shown to reduce the risk of developing depression in children and adults.

Though the seasons and our life circumstances may change, our basic needs do not. Just as we shift from shorts to coats or fresh summer fruits and vegetables to heartier fall food choices, so too must we shift our seasonal approach to how we stay physically active. Some of that is simply adapting to conditions: bundling up for a walk, wearing the appropriate shoes, or playing in the snow with the kids instead of playing soccer in the grass.

Sometimes there’s a bit more creativity involved. Often this means finding ways to simplify activity or make it more accessible. For example, it may not be possible to get to the gym or even take a walk due to weather or any number of reasons. In those instances, other options include adding new types of movement — such as impromptu dance parties at home — or doing a few household chores (yes, it all counts as physical activity).

During the COVID-19 pandemic, I built a makeshift gym in my garage as an alternative to driving back and forth to the gym several miles from home. That has not only saved me time and money but also afforded me the opportunity to get 15 to 45 minutes of muscle-strengthening physical activity in at odd times of the day.

The point to remember is that no matter the approach, the Physical Activity Guidelines recommend that adults get at least 150 minutes of moderate-intensity aerobic activity (anything that gets your heart beating faster) each week and at least 2 days per week of muscle-strengthening activity (anything that makes your muscles work harder than usual). Youth need 60 minutes or more of physical activity each day. Preschool-aged children ages 3 to 5 years need to be active throughout the day — with adult caregivers encouraging active play — to enhance growth and development. Striving toward these goals and then continuing to get physical activity, in some shape or form, contributes to better health outcomes both immediately and over the long term.

For youth, sports offer additional avenues to more physical activity and improved mental health. Youth who participate in sports may enjoy psychosocial health benefits beyond the benefits they gain from other forms of leisure-time physical activity. Psychological health benefits include higher levels of perceived competence, confidence, and self-esteem — not to mention the benefits of team building, leadership, and resilience, which are important skills to apply on the field and throughout life. Research has also shown that youth sports participants have a reduced risk of suicide and suicidal thoughts and tendencies. Additionally, team sports participation during adolescence may lead to better mental health outcomes in adulthood (e.g., less anxiety and depression) for people exposed to adverse childhood experiences. In addition to the physical and mental health benefits, sports can be just plain fun.

Physical activity’s implications for significant positive effects on mental health and social well-being are enormous, impacting every facet of life. In fact, because of this national imperative, the presidential executive order that re-established the President’s Council on Sports, Fitness & Nutrition explicitly seeks to “expand national awareness of the importance of mental health as it pertains to physical fitness and nutrition.” While physical activity is not a substitute for mental health treatment when needed and it’s not the answer to certain mental health challenges, it does play a significant role in our emotional and cognitive well-being.

No matter how we choose to be active during the holiday season — or any season — every effort to move counts toward achieving recommended physical activity goals and will have positive impacts on both the mind and the body. Along with preventing diabetes, high blood pressure, obesity, and the additional risks associated with these comorbidities, physical activity’s positive effect on mental health is yet another important reason to be active and Move Your Way .

Importance of Physical Fitness Essay

People all over the world are becoming fitness conscious as they have realized that it is the most important and have shifted their priorities from everything else to keep them fit and fine as healthy with wealth. Regular exercise has several benefits it helps the body to get into the desired shape and develops resistance power from gaining additional and excessive weight. It helps a person safe from diseases such as obesity, blood pressure, heart diseases, and high cholesterol. Furthermore, an activity also helps in reducing depression and insomnia by enhancing sleeping routines. Also, such activity is known to produce positive vibes inside a person which in turn increases the person’s confidence. (Hardcastle, 2006)

Fitness training fine-tunes the muscles using and burning calories in the body which also increases the metabolic rate in the body. While an inactive person will have his or her body losing its muscles decreasing the metabolism of the body, resulting in concentration of calories and formation of fat and the person gains weight. Exercises like walking, jogging, running, swimming, doing push-ups and pull-ups, running up a flight of stairs, doing crunches, cycling, etc. not only helps a person maintain an outer strong and toned look but it also help the organs like heart, lungs, liver and other to function properly while giving mental peace. For the community, there can be several initiatives taken to help people exercise regularly and maintain a healthy diet. Being a nutritionist advisor, I can guide people on what types of food are healthy and which types of foods should be avoided. As a social welfare worker, I can organize marathons for social causes where people can participate in the form of marathon exercises and also serve a more worldly cause. By adopting an educationist’s role, I can educate the positive aspects of personal fitness and make people aware of the negatives of not exercising. Also, I can start dancing lessons for young boys and girls where from an early stage not only children learn to dance but also learn to exercise. Furthermore, I can become a sport’s proponent where teaching people to play sports will also help them increase their activity and become fit. (Personal Health Zone, 2009)

The most common type of exercise is walking or running. From a casual stroll to a paced walk every day works the muscles relentlessly. Yet another popular exercise common amongst the younger people is playing sports since most sports involve running and stretching out the body muscles. Aerobics or even dancing is a type of exercise that is fun yet physically strengthening. Although swimming might fall under the banner of sports, most people swim to relax in the water and have a good time, and can exercise their body at the same time. For the more body-conscious people, bodybuilding is a set of exercises that strengthens the muscles by picking weights and putting regular pressure. Carrying excessive loads from to and fro also is a form of exercise. Furthermore, in a computer-related work environment, where repetitive strain injury (RSI) is quite common, people should take regular breaks and stretch out their muscles. People recommend the use of machines available in gymnasiums to flex their muscles. Yet another method of exercising is cycling either on a bicycle or a gymnasium machine. (ADAM, 2004)

Active people tend to outlive those who are inactive. In case of being physically inactive for a long time and then taking drastic measures to lose weight to achieve a good result is not an efficient way, as it will not have a lifelong effect on the body. It is rather recommended to consult the physician and go slowly and step-wise. Alongside maintaining a healthy lifestyle by eating healthy food, avoiding junk and fast food, and exercising regularly mainly in the early morning and late evening as these are the effective times for exercising.

Healthy Lives and Well-being for Everyone: Why SDG 3 matters and how we can achieve it

Creating a sustainable world—and reaching economic, environmental and social goals—depends on having a thriving and healthy human population. We explore how Sustainable Development Goal 3 plans to achieve that.

Creating a sustainable world—and reaching economic, environmental and social goals—depends on having a thriving and healthy human population.

However, even the most cursory glance at figures pertaining to human health reveals a world where grave inequalities result in massive disparities when it comes to access to basic health care, and where easily treatable diseases still claim far too many lives in many corners of the globe.

Let’s start at birth. In 2015, there were approximately 303,000 maternal deaths worldwide, most from preventable causes. Maternal health conditions were also the leading cause of death among girls aged 15-19 in that year.

Infant mortality rates—along with many other health-related issues—can also expose inequalities within nations. In Canada, for example, while the average mortality rate is around 5 deaths per 1,000 live births, it reaches as high as 16 deaths per 1,000 live births in Nunavut—a region where 85 per cent of the population is Indigenous.

Around the world, more than 6 million children still die before their fifth birthday each year, with four out of five of those deaths occurring in sub-Saharan Africa and Southern Asia. Rates of poverty and the level of the mother’s education are key factors that affect the likelihood of a child making it past the age of five.

When it comes to communicable diseases, at the end of 2013, there were an estimated 35 million people living with HIV worldwide. In fact, 240,000 children were newly infected with the disease that year.

While the rates of malaria are falling globally, the recent resurgence of ailments such as measles and the Zika virus reminds us that there are always potential health crises around the corner for which we may not be equipped—with the Global South often most at risk.

A Global Approach to Worldwide Problems

When world leaders adopted the Sustainable Development Goals, they signed on to a goal (SDG 3) that aims to "ensure healthy lives and promote well-being for all at all ages."

The Millennium Development Goals (MDGs), which provided a global framework for development from 2000-2015, and dedicated a hefty 3 out of 10 goals to global health issues (child mortality; maternal health; HIV/AIDS, malaria and other diseases).

The targets under SDG 3 have an even greater scope than those three MDGs combined. Furthermore, given the integrated nature of the sustainable development approach, many of the other SDGs, such as Goal 1 (“end poverty”), Goal 2 (“end hunger”) and Goal 6 (“ensure access to water”), are strongly tied to—and have an impact on—human health issues.

The recent resurgence of ailments such as measles and the Zika virus reminds us that there are always potential health crises around the corner for which we may not be equipped—with the Global South often most at risk.

Many of the SDG 3 targets are dedicated to tackling pressing issues surrounding maternal health and child mortality rates, which continue to affect much of the Global South in particular. The ambition of those targets reflects the urgency of the work at hand, and the desire of the international community to continue their work on the unfinished business in the MDGs. By 2030, Target 3.2 aims to “end preventable deaths of newborns and children under 5 years of age,” and Target 3.1 is to “reduce the global maternal mortality ratio to less than 70 per 100,000 live births.”

Other targets reflect the universal nature of the SDGs. They touch on everything from universal health coverage and tobacco control to reducing the number of deaths due to road traffic accidents and substance abuse—issues that are widespread in countries at all stages of development.

Both developed and developing countries have work to do to ensure healthy lives and promote well-being for all of their citizens, including addressing policies on universal access to health coverage and populations’ relationships with alcohol and narcotics.

Towards a Healthier Future

The global community has already made significant progress in key areas of human health. Despite the still-high figures of maternal mortality, the United Nations reports it has actually fallen by almost 50 percent since 1990. In Northern Africa and Southern and Eastern Asia, maternal mortality has also been reduced by around two-thirds.

17,000 fewer children die each day than in 1990—some of which can be attributed to increased access to vaccinations. For example, since 2000, the United Nations reports that measles vaccines have prevented almost 15.6 million deaths globally.

When it comes to treating HIV, at the end of 2014, 13.6 million had access to antiretroviral therapy, and new HIV infections in 2013 were estimated at 2.1 million—38 per cent lower than in 2001.

These achievements point to the value of international goals—the MDGs—in focusing global efforts on shared objectives. The SDGs continue, and widen the scope of actors and efforts, in order to ensure that no one is left behind due to lack of access to health care and healthy lifestyle options.

What is brain health and why is it important?

Yongjun Wang and colleagues discuss the definition of brain health and the opportunities and challenges of future research

The human brain is the command centre for the nervous system and enables thoughts, memory, movement, and emotions by a complex function that is the highest product of biological evolution. Maintaining a healthy brain during one’s life is the uppermost goal in pursuing health and longevity. As the population ages, the burden of neurological disorders and challenges for the preservation of brain health increase. It is therefore vital to understand what brain health is and why it is important. This article is the first in a series that aims to define brain health, analyse the effect of major neurological disorders on brain health, and discuss how these disorders might be treated and prevented.

Definition of brain health

Currently, there is no universally recognised definition of brain health. Most existing definitions have only a general description of normal brain function or emphasise one or two dimensions of brain health. The US Centers for Disease Control and Prevention defined brain health as an ability to perform all the mental processes of cognition, including the ability to learn and judge, use language, and remember. 1 The American Heart Association/American Stroke Association (AHA/ASA) presidential advisory defined optimal brain health as “average performance levels among all people at that age who are free of known brain or other organ system diseases in terms of decline from function levels, or as adequacy to perform all activities that the individual wishes to undertake.” 2

The brain is a complex organ and has at least three levels of functions that affect all aspects of our daily lives: interpretation of senses and control of movement; maintenance of cognitive, mental, and emotional processes; and maintenance of normal behaviour and social cognition. Brain health may therefore be defined as the preservation of optimal brain integrity and mental and cognitive function at a given age in the absence of overt brain diseases that affect normal brain function.

Effect of major neurological disorders on brain health

Several neurological disorders may disrupt brain function and affect humans’ health. Medically, neurological disorders that cause brain dysfunction can be classified into three groups:

Brain diseases with overt damage to brain structures, such as cerebrovascular diseases, traumatic brain injury, brain tumours, meningitis, and communication and sensory disorders

Functional brain disorders with detectable destruction of brain connections or networks, such as neurodegenerative diseases (eg, Parkinson’s disease, Alzheimer’s disease, and other dementias) and mental disorders (eg, schizophrenia, depression, bipolar disorder, alcoholism, and drug abuse)

Other brain disorders without detectable structural or functional impairment, such as migraine and sleep disorders.

These neurological disorders may have different or common effects on brain health and function. For instance, Alzheimer’s disease is the main type of dementia, with a decline in different domains of cognitive function. Mood disorders may cause dysfunction in execution, reward processing, and emotional regulations. In addition to physical disability, aphasia, gait and balance problems, and cerebrovascular diseases may lead to cognitive impairment and dementia, which are neglected by both patients and physicians.

Ageing and burden of neurological disorders

Human ageing is mainly reflected in the aspects of brain ageing and degradation of brain function. The number of people aged 60 years and over worldwide was around 900 million in 2015 and is expected to grow to two billion by 2050. 3 With the increases in population ageing and growth, the burden of neurological disorders and challenges to the preservation of brain health steeply increase. People with neurological disorders will have physical disability, cognitive or mental disorders, and social dysfunction and be a large economic burden.

Globally, neurological disorders were the leading cause of disability adjusted life years (276 million) and the second leading cause of death (9 million) in 2016, according to the Global Burden of Diseases study. 4 Stroke, migraine, Alzheimer’s disease and other dementias, and meningitis are the largest contributors to neurological disability adjusted life years. 4 About one in four adults will have a stroke in their lifetime, from the age of 25 years onwards. 5 Roughly 50 million people worldwide were living with dementia in 2018, and the number will more than triple to 152 million by 2050. 6 In the following decades, governments will face increasing demand for treatment, rehabilitation, and support services for neurological disorders.

Opportunities and challenges of future research on brain health

Opportunities and challenges exist in the assessment of brain health, the mechanism of brain function and dysfunction, and approaches to promote brain health ( box 1 ).

Lack of metrics or tools to comprehensively assess or quantify brain health

Little knowledge about the mechanisms of brain function and dysfunction

Few effective approaches to prevent and treat brain dysfunction in some major neurological disorders, such as dementia

Need to precisely preserve brain functions for people with neurosurgical diseases

Defining and promoting optimal brain health require the scientific evaluation of brain health. However, it is difficult to comprehensively evaluate or quantify brain health through one metric owing to the multidimensional aspects of brain health. Many structured or semistructured questionnaires have been developed to test brain health by self-assessments or close family member assessments of daily function or abilities. In recent decades new structural and functional neuroimaging techniques have been applied to evaluate brain network integrity and functional connectivity. 7 However, these subjective or objective measures have both strengths and weaknesses. For instance, scales such as the mini-mental state examination and Montreal cognitive assessment are simple and easy to implement but are used only as global screening tools for cognitive impairment; tests such as the digit span, Rey-Osterrieth complex figure test, trail making A and B, Stroop task, verbal fluency test, Boston naming test, and clock drawing test are used mainly to assess one or two specific domains of memory, language, visuospatial, attention, and executive function; and neuroimaging techniques, although non-invasive and objective, still have disadvantages of test contraindications, insufficient temporal or spatial resolution, motion artefact, and high false discovery rates, which limit their clinical transformation.

Another difficulty in measuring brain health is that age, culture, ethnicity, and geography specific variations exist in the perception of optimal brain health. Patient centred assessment of brain function, such as self-perception of cognitive function and quality of life, should also be considered when measuring brain health. 8 Universal acceptable, age appropriate, multidimensional, multidisciplinary, and sensitive metrics or tools are required to comprehensively measure and monitor brain function and brain health.

To promote optimal brain health, we need a better understanding of the mechanisms of brain function and dysfunction. Unfortunately, little is known about the working mechanism of the brain. Although we have made considerable developments in neuroscience in recent decades, we still cannot totally decipher the relations between spatiotemporal patterns of activity across the interconnected networks of neurons and thoughts or the cognitive and mental state of a person. 9 Recent progress in brain simulation and artificial intelligence provides a vital tool to understand biological brains, and vice versa. 10 11 The development of brain inspired computation, brain simulation, and intelligent machines was emphasised in the European Union and China Brain Project. 9 12

Meanwhile, the mechanisms behind the brain dysfunction in some neurological disorders are still not well understood, especially for mental and neurodegenerative disorders. Further investigation of the mechanisms of brain diseases may indicate approaches to treatment and improve brain function. Brain imaging based cognitive neuroscience may unravel the underlying brain mechanism of cognitive dysfunction and provide an avenue to develop a biological framework for precision biomarkers of mood disorders. 13

Most common neurological diseases, such as cerebrovascular diseases and Alzheimer’s disease, have complex aetiopathologies, typically involving spatial-temporal interactions of genetic and environmental factors. However, a single genetic factor could account for the disease progression of monogenic neurological disorders. These diseases could be more readily investigated by simplified cross species modelling, leading to better understanding of their mechanisms and greater efficiency in testing innovative therapies. Such research may provide a window to promote the investigation of common neurological disorders and general brain health, as discussed by Chen and colleagues elsewhere in this series. 14

Few effective approaches are available to prevent and treat brain dysfunction in some major neurological disorders, such as dementia. Neurons are not renewable, and brain dysfunction is always irreversible. Recent trials targeting amyloid clearance and the selective inhibition of tau protein aggregation failed to improve cognition or modify disease progression in patients with mild Alzheimer’s disease. 15 16 More attention has focused on other potential therapeutic targets, such as vascular dysfunction, inflammation, and the gut microbiome, as discussed by Shi and colleagues. 17 In particular, recent studies showed that the early impairment of cognition was induced by the disruption of neurovascular unit integrity, which may cause hypoperfusion and the breakdown of the blood-brain barrier and subsequent impairment in the clearance of proteins in the brain. 18 19 Physical activity, mental exercise, a healthy diet and nutrition, social interaction, ample sleep and relaxation, and control of vascular risk factors are considered six pillars of brain health. The AHA/ASA presidential advisory recommended the AHA’s Life’s Simple 7 (non-smoking, physical activity, healthy diet, appropriate body mass index, blood pressure, total cholesterol, and blood glucose) to maintain optimal brain health. 2 Pan and colleagues discuss how this may indicate a new dawn of preventing some cognitive impairment and brain dysfunction by preventing vascular risk factors or cerebrovascular diseases. 20

For other neurological disorders with potential therapeutic approaches, the main aim is to preserve brain function. Impaired brain function due to anatomical structural damage is underestimated in patients with neurosurgical diseases such as brain tumours, trauma, and epilepsy. In recent years, treatment targets for neurosurgical diseases have changed from focusing on survival or life expectancy to balancing brain structures and functions. Precise preservation of brain function requires an understanding of the exquisite relation between brain structure and function and advanced technologies to visualise brain structure-function relations. 21

Another example of the predicament associated with protection of brain function is uncertainty in the treatment response in epilepsy management. Current standard care for epilepsy relies on a trial and error approach of sequential regimens of antiseizure medications. The time delay due to this treatment approach means that such treatments may be less effective and irreversible damage may occur. Chen and colleagues 22 describe how recent advances in personalised epilepsy management based on artificial intelligence, genomics, and patient derived stem cells are bringing some hope to overcome this predicament in epilepsy management and promise a more effective strategy. 23 24

Brain health is the maintenance of multidimensional aspects of brain function. However, several neurological disorders may affect brain health in one or more aspects of brain function. Deciphering and promoting the function and health of the brain, the most mysterious organ in the human body, will have a dramatic impact on science, medicine, and society. 25 In the past seven years, a number of large scale brain health initiatives have been launched in several countries to promote the development of neuroscience, brain simulation, and brain protection. 9 However, further challenges are raised by the different key research directions of brain projects in different countries. In the face of these challenges, Liu and colleagues argue that collaboration on brain health research is urgently needed. 26 As the other articles in this series describe, coordinated research has enormous potential to improve the prognosis of brain disorders.

According to the Food Standards Agency, a healthy diet for most people consists of fresh fruits and vegetables, whole grain foods, lean proteins and some low fat or nonfat dairy products. It's just as important to limit foods that are high in fat, sugar, sodium and cholesterol as it is to choose healthy foods.

The Centers for Disease Control and Prevention notes that exercise and diet play dual roles in maintaining healthy weight and aiding weight loss. In addition, people's hearts grow stronger and healthier when they engage in cardiovascular exercise that raises heart rates for an extended time.

The Cleveland Clinic suggests that a healthy, nutrient- and antioxidant-rich diet also guards against cancer and helps improve the immune system's functions. Finally, the CDC notes that people of all ages, ethnicities, sizes and shapes have the power to extend the length of their lives by regularly engaging in physical activity.

Mental Benefits

Eating well and getting proper exercise offer mental benefits as well as better physical health. HelpGuide.org notes that healthy, mindful eating practices can help reduce stress in adults' lives, as can mild physical activities, such as walking, yoga and tai chi. Exercise stimulates brain chemicals that make people happy. Also, the CDC points out that a healthy lifestyle helps sharpen thinking, learning and judgment skills, and reduces the risk of depression.

No matter how old a person is or what kind of diet and exercise pattern he or she follows, it's possible for that person to make adjustments and improve health. A person who is not very active can start by making small changes, such as replacing soft drinks with water and adding a 10-minute walk to a daily routine.
