IOSH Managing Safely is one of the most popular health and safety certificates in the world. It's designed to bring you up to speed on your health and safety responsibilities and help you develop the practical skills to manage risk on a day-to-day basis. To achieve your certificate, you need to pass two assessment:

  • A knowledge-based, multiple-choice online test
  • A skills-based, practical risk assessment project

In this guide, we've provided step-by-step advice on how to successfully completed and submit your practical risk assessment project. However, the IOSH Managing Safely also includes a multiple choice exam.  

What is the IOSH Managing Safely risk assessment?

The IOSH Managing Safely risk assessment project is the practical section of the IOSH Managing Safely course. It tests your ability to apply what you’ve learned in a real-life situation.

You’re required to carry out a risk assessment of a workplace of your choice. You need to identify 4 separate hazards from the categories covered in the IOSH Managing Safely syllabus: 

  • Environmental
  • Organisational

For each of your identified hazards, you need to decide:

  • The likelihood of an event occurring
  • Who might be harmed
  • Control measures 

You then need to record your findings in a table that’s provided to you – that’s it! We’ve broken down exactly what to include in each of these sections further below. 

When do you complete the IOSH Managing Safely risk assessment? 

You need to submit your IOSH Managing Safely project within 14 days of finishing your course. If you’re studying in a classroom or virtual classroom, your 14 days will start from your course’s last day. If you’re studying online, your 14 days will start from when you’ve completed all of your learning materials. 

How long should it take to complete the IOSH Managing Safely risk assessment? 

Unlike written exams, practical assignments don’t have a time limit, so that you can take as long as you! We always recommend to work through your assessment in one go, then go back and re-read it a day or two later with a fresh pair of eyes and make any amendments.   So long as you focus, it shouldn’t take you more than 2 hours to assess your environment and write up your findings in the table provided.

What are the pass marks for the IOSH Managing Safely risk assessment?

You’re required to identify 4 hazards. For each hazard and the decision you make, you can gain a mark. You can also gain marks by filling in the introduction and review date of your assessment – these are easy marks to gain, so don’t forget to fill in these sections! There are 38 marks available in total, and you need to achieve 23 (60%) to pass. 

IOSH Managing Safely Marking Criteria

Getting started with your iosh managing safely risk assessment.

Choose a suitable workplace that has a range of hazards available. You need to identify hazards from a range of categories. If your own work doesn’t do, then get in touch with your course provider, and they’ll source you something more suitable. Make sure you’ve marked the submission deadline in your diary. If you don’t submit your assessment on time, then you’ll fail. Please don’t leave it until the last minute! Make sure you read ALL of the guidance given to you by your course provider.

IOSH Managing Safely risk assessment stages

The risk assessment process is broken down into stages. Each stage should flow together to tell an inclusive story of your workplace, and it’s hazards, risks, and control measures. 

Make sure you fill in ALL of the boxes available on the Risk Assessment Form because they earn you marks, and let’s face it, every mark counts!

Fill in your details – (1 mark)

The first mark available is an easy one. Set the scene for your assessor by filling in the following:  

  • Name – Your Name
  • Course End Date – When you finished your course
  • Training Provider – Who delivered your training 
  • Assessors Name – Your Name
  • Date and Time – state when you undertook the assessment
  • Work area – where the task is being undertaken
  • The task being assessed – what task you are risk assessing. For example, cleaning up spilt oil, carrying boxes from a conveyor belt to a pallet, or cutting pieces of wood with a circular saw.

Clearly Identify the Hazard (1 Mark)

The first column is where you identify the hazard. A hazard is defined as “anything that has the potential to cause harm.”   Make sure that you clearly describe what the hazard is. This doesn’t mean you have to write a lot, but what you do put down should clarify the hazard. Remember, you would distribute a risk assessment to key personnel in real life, so you need to be as clear as possible.     For example, “ slip hazard ” is rather vague. However, “A  slip hazard due to spillages of oil/petrol” provides a bit more clarity for the marker and workers who read the risk assessment.

Who Might be Harmed? (1 Mark)

You’re likely to automatically think of the worker(s) undertaking the task and the workers within the immediate area. But remember that others might also be at risk. Think outside the box. Do you have visitors in the work area? Cleaners? Maintenance workers? The public? If so, they should be noted down here too.

How Might People be Harmed? (1 Mark)

This includes injury and ill health. We often think of the immediate effects of hazards in terms of injuries. But it would be best if you also considered whether there are any long term ill-health effects associated with the hazard. Clearly state what any injury or ill-health effects are in this section.

Existing Risk Control Measures (1 Mark)

This part asks you to identify what measures of control you have in place for this hazard at the time of your assessment.   Don’t limit your answer to physical controls such as guards, local exhaust ventilation (LEV), signage etc. Consider administrative/soft controls such as training, supervision, job rotation, or safe work systems. (These are also important to consider when you get to the Additional Controls stage below).   Be clear with your description of the controls. Don’t just write one-word controls such as “PPE”. Clearly define what type of PPE is required. For example, nitrile gloves or safety goggles.

Current Risk Rating (1 Mark)

This is where you rate the risk of the hazard with the controls that are currently in place. You do this using the IOSH 5 x 5 Risk rating calculator.

The IOSH 5 x 5 risk rating calculator rates the likelihood and consequences on a scale of 1-5:

  • L stands for Likelihood – how likely is it that the hazard will harm someone?
  • C stands for Consequence – what are the consequences if someone is harmed? This is also often referred to as the Severity of harm.

Once you have identified the likelihood and consequence, you multiply the two numbers to get your Risk Rating (R).   Likelihood x Consequence = Risk   For example, if you think the likelihood is ‘Likely’ (4) and the Consequence is ‘Moderate’ (3), then:

4 x 3 = 12. Your Risk Rating is 12. Lots of learners stress over getting this number right. Don’t worry; the marker is fully aware that this type of rating system is subjective. There will always be a variety of opinions with the risk rating. Just make sure that you are realistic with your ratings, and you’ll be all right.

Additional Controls (1 Mark)

This requires you to provide additional controls that will reduce the risk. Be realistic and take into account the risk rating you just calculated.   As discussed in the section’ Existing Risk Control Measures’ above, make sure you think about various controls, not just physical ones.

Residual Risk Rating (1 Mark) 

Once you’ve decided on your new control measures, you need to carry out another risk rating using the same IOSH 5 x 5 risk rating calculator.     Your new Residual Risk Rating should be lower than your previous one. 

Action Monitored by Whom? (1 Mark)

State who is to monitor each additional control by job role—for example, Engineering Manager. 

Action Monitored by When? (1 Mark)

There’s no plan without timelines, so allocate a time scale for each additional control. Don’t put phrases such as “On-going” or “Within a few weeks”. Use timely, measurable timeframes such as 2 weeks, 6 weeks, 2 months.   Make sure you follow the IOSH Action Level Table that provides you with guidance based on your Risk Rating:

Review Date, Signature and Training Provider (1 Mark)

Another easy mark! Sign off your risk assessment by filling in the details at the bottom of the form outlined below:

  • Review date: make sure that you give a realistic review date for your assessment, for example, 12 months.
  • Sign the Assessment – if sending via computer your printed name.
  • Put your Training Provider name in the final box.

How to submit your IOSH Managing Safely risk assessment

You need to email us your finished assignment within your 14-day deadline. We’ll send your work to IOSH for marking straight away.

IOSH will typically send us the result of your practical assignment within 2-3 weeks of submission. We’ll let you know if you’ve passed or failed as soon as we hear from them.   Remember, you also need to pass the multiple-choice knowledge test to achieve your full IOSH Managing Safely Certificate.   Provided you’ve passed both assessments, we’ll send you your hard-copy IOSH Managing Safely certificate in the post within 6-8 weeks of completion.

What is IOSH Managing Safely Risk Assessment  

Administered by the IOSH, Managing Safely is the authority course on health and safety. Risk assessment in IOSH Managing Safely Risk Assessment is a systematic procedure that assists organisations in identifying, evaluating, and controlling workplace dangers to prevent accidents and injuries.   

Risk assessment is a crucial component of the IOSH Managing Safely course. It provides managers and supervisors with the skills and knowledge they need to identify possible hazards, assess the risks and take action to eliminate or control them.   

The IOSH Managing Safely Risk Assessment is a practical project part of the IOSH Managing Safely course. This section includes a practical risk assessment where you need to identify hazards and safety risks. You need to identify four hazards at your workplace and list the following things:  

1) Who might get harmed and how?  

2) What are the control measures that are already in place?  

3) Current risk rating  

4) What are some of the additional controls that can be added?  

5) How do the new controls change the risk rating?  

6) When and who will implement these new controls?   

Stages of IOSH Managing Safely Risk Assessment

Basic assessment details  

This is given at the top of the risk assessment form and contains all the basic details about the individual carrying out the assessment (assessor). Let’s look at each of the data that needs to be filled in:  

a) Name of assessor: This refers to your name; not the IOSH Managing Safely examiner.  

b) Date and time of the assessment  

c) Work Area: This refers to the area where you will carry out the assessment. It could be a warehouse, office or a workshop. Do not just mention the place vaguely. Describe the place in brief. For example, a workshop that is 700 square meters with 2 CNC machines (can also describe the number of people)  

d) Task being assessed: Be specific about the situation that want to assess. For example, you are assessing the operator using a CNC machine and storing the finished product.  

For the first section, you need to plan out the work area and task being assessed before going into the assessment. Pick an area and situation where there are hazards that you can assess and control them. This section carries a total of one mark. 

What is the Hazard  

This section details a hazard. Hazard is defined as something that has a potential to cause harm. You need to select four hazards that are not being controlled appropriately. Each hazard carries one mark. 

Make sure that the hazard you are describing is not too vague. For example, instead of “tripping”, it should be “trip hazards because of trailing wires”.  It is critical to understand that the hazard is not necessarily what directly causes harm. Taking the same example, “tripping” is not a hazard but “trailing wires” or “wet floor” is a hazard. Therefore, do not be too vague with the hazards you mention. 

Understand the difference between hazards and risks, learn how to identify hazards with the IOSH Working Safely Training .  

Who might be harmed?  

After evaluating the hazards in the workplace, the assessor must determine who may be harmed by those hazards. This section identifies the group of people who may be exposed to the hazards listed in the previous section.  

The assessor must consider everyone who may be at the risk of each hazard. Employees, visitors, janitors, or any other group of people who may be at risk must be included. The assessor should evaluate whether certain groups are more vulnerable to the hazard than others.  

For example, an unsuspecting visitor might be at a greater risk of tripping over trailing wires or slip on leaked oil. If the identified hazard is a piece of machinery at a factory, the assessor must consider whether employees may be harmed by utilising the machinery, as well as any visitors or contractors who may come into contact with the machinery.  

How might people be harmed?  

In this section, you need to detail the nature of injury that the identified hazard could cause. There is no need to go into too much detail. Mentioning the nature of injury will suffice. In some cases, hazards can cause death which needs to be mentioned as well. 

For example, falls from tripping over wire can cause bruise, concussion or broken bones. Blades on heavy equipment like standing drill or CNC can cause amputation or serious cuts. Do not exaggerate or undercut the severity of the injury caused by the identified hazard. Be precise about it. 

Existing risk control measures  

In this section, you need to mention the existing systems or control measures in place. For example, a signage near the wet floor can warn people or having set of processes and training in place to avoid mishaps with the heavy machineries in a workshop. Whatever controls measures you have in place must be mentioned here. 

Risk Rating

The Risk Matrix   

The risk matrix combines the ‘likelihood’ and ‘consequence’ of each hazard to provide a risk rating. The hazard’s likelihood (denoted by ‘L’ on the assessment form) is assessed on a scale of 1 to 5, with 1 being unlikely and 5 being extremely likely. The hazard’s consequence (denoted by ‘C’ on the assessment form) is also assessed on a scale of 1 to 5, with 1 being minor and 5 being severe. The risk rating is calculated by multiplying the likelihood and consequence scores.   

For example, if the hazard’s likelihood is 3 and the consequence is 4, the risk rating is 12 (3 x 4). The risk rating can be used to prioritise the threats and determine which hazards must be addressed first. The highest risk has a rating of 20-25 and calls for immediate action and stopping the associated activity. High-risk has a rating of 15-16 and calls for immediate action and stopping the activity if required.  

Medium risk has a rating of 8-12 and calls for improvement in control measures under a strict timeline. Low-risk danger has a rating of 3-6 and calls for minor improvement in the next review or if there is a significant change. Any rating lower than this doesn’t call for any action, but it must be ensured that controls are maintained.  

Additional controls   

This section is designed to identify extra control measures that can be taken to decrease the risks associated with the hazards identified. These new measures are known as additional controls.  

The assessor must consider all possible control measures that can be implemented to reduce the risk rating associated with each hazard in this section. For example, replace a hazardous chemical with a less dangerous one or use machine guards to prevent access to moving parts in heavy equipment.  

Management controls such as altering the way work is done to decrease the risk of harm, limiting exposure to a hazard, and supervision and safe systems of work can reduce risk rating. Another example can be the use of protective equipment like gloves, goggles, or respirators, for example, to protect against chemical exposure or airborne particles.

New Risk Rating   

After implementing the additional control measures, the assessor should use the risk matrix to update the risk level associated with the hazard. If the added controls are effective, the new risk rating should be lower than the previous risk rating. If the control measures are ineffective or insufficient, the new risk rating may be maintained or even raised.  

The assessor should check that the risk has been reduced to an acceptable level by the control mechanisms put in place. If the new risk rating is still high, the assessor should explore additional control measures to further reduce the risk rating.  

However, you need to be careful with the consequence and likelihood rating. It is much easier to reduce the likelihood rating than the consequence rating. For example, placing machine guards can reduce the likelihood of getting cut by blades, but the consequence can still be amputation or severe cuts.  

It is possible to reduce the consequence rating as well, but it is not always possible. For example, replacing a chemical with a less volatile or less dangerous chemical can do less harm, although such measures are not always practical.  

Action/Monitored by whom?   

In this section, you need to mention who will be responsible for the implementation of the new control measures. For example, an operations or HR manager or a site supervisor can be mentioned in this section.  

Action/Monitored by when?   

This section is to mention the dates or deadlines by which the new measures must be implemented. Please note that hazards with high-risk ratings need to be addressed immediately while hazards with low or medium-risk ratings can be implemented with a more flexible deadline. It is also important to be practical about the dates you mention.   

Closing details   

The closing detail is the final section of the form and contains the ‘review date’ section. This refers to a set date on which identified hazards on the risk assessment are reviewed to check the effectiveness of new control measures.

IOSH Managing Safely Risk Assessment Stages

We’ll now break down each section of the risk assessment form with clear guidance on how to fill in each one, so you have a thorough understanding of how to tackle your risk assessment.

Personal details and assessment information:

As mentioned earlier, the first mark is awarded for filling in the personal details at the top of the risk assessment form. This includes the name of the assessor. As mentioned earlier, this refers to the individual carrying out the risk assessment, not the IOSH Managing Safely examiner. You therefore need to put your name, as you are the one carrying out the risk assessment. This section also includes the date and time, as well as the ‘Work Area’. For this you need to describe the sort of work environment you are conducting the risk assessment in, such as an office, warehouse, or workshop; you don’t need to give specific details about the organisation which operates in the workplace. Finally, there’s the ‘Task being assessed’ section, this refers to the hypothetical task a worker would be carrying out and could be something as simple as cleaning the office or installing a smoke detector. You’ll need to decide on this task before beginning the risk assessment, as by going through the actions involved in the task you can identify the risks that would be involved in completing it.

We’ll now move from left to right across the form, covering each section so that you gain an understanding of how each part leads into the next.

What is the hazard?

The first section asks you to identify the hazard you will be assessing the risk of. When filling in this section, bear in mind that a hazard may not be what directly causes harm, instead being what leads to harm being caused. For example, electricity is what causes harm through an electric shock, whereas the hazard would be a damaged plug or cable, as this leads to an electric shock occurring. A hazard is also not an event, such as tripping, slipping, or falling. In these situations, the hazards would be what causes the event, being the loose cable and spilled liquid respectively.

Who might be harmed?

After identifying the potential hazard, decide which people in a business could be at risk of harm from the hazard. This will likely include employees, though may also include contractors, visitors, the public, or other groups present in your workplace. Make sure to include all those who could be at risk of harm from the hazard.

How might people be harmed?

For this section you will need to list what injuries the identified risk could cause. You don’t need to go into explicit detail, though you will need to be thorough in covering all the possible injuries. Here are a few examples:

Electric shocks can cause electric burns and interfere with the natural rhythm of the heart.

Falls can cause bruising, concussion, and broken bones.

Metal blades on equipment can cause cuts and lead to amputation.

4 Expert Tips For Filling Out The IOSH Risk Assessment Form

The IOSH risk assessment form is part of the formal assessment for the IOSH Managing Safely qualification.

The risk assessment form puts everything you’ve learnt on the course to the test and needs to be filled out within a fortnight of the course end. You have two hours to clearly highlight four hazards in your workplace and how you would deal with them.

Marked out of 38, you need a score of 23 to pass. At first, it may seem daunting, but never fear! Here are four expert tips for filling it out. Follow these guidelines and all your hard work on the course will be worth it!

1. Remember the examiner is unfamiliar with your workplace

Always keep in mind that the person marking your IOSH risk assessment form is unlikely to know what your place of work looks like. So you need to put all your information into context for them. Be very clear and concise when describing the area you are examining.

The top section of the form where you fill out basic information like the date and time is worth one mark so should be very easy! Just don’t fall into the trap of thinking that minimal marks means minimal information.

Under ‘Work Area’ and ‘Task Being Assessed’, provide details that will help your examiner to better understand what you are looking at.

Include extra details about the nature of the work area, for example, how many people work in it, its size and what the area is used for. When describing the task you’re exploring, be specific here too. If you are monitoring employees taking down scaffolding from a five-story office building, include information about the building, the surroundings, the ground level environment, the weather and so on. Don’t just write Dismantling scaffolding and think that’s enough.

Your description should be comprehensive enough to offer your examiner enough detail so they can visualise the situation and put your identified hazards and recommendations into context.

Also, keep in mind that the form will have a word limit, so be as concise in your detail as possible. Get someone else to proofread your form before submitting it to catch any vague descriptions.

2. Make sure your identified hazards are actually hazards.

The second section comprises of four rows, one for each of the four hazards you identify. There are nine columns in each row, all worth one mark each. Here’s the catch though. If you put a hazard in the first column that isn’t actually a realistic hazard, no matter what you write in the rest of that row, you’ll lose all nine points because you aren’t assessing a real risk.

So, expert tip number two is to make sure your identified hazards are actually real hazards!

The point of the IOSH risk assessment is to see how you identify and address risks so if you don’t properly identify a risk, you’ve fallen at the first hurdle.

When you outline your four hazards, make sure that your description is rich. If the hazard is that someone might fall from a height when working on scaffolding, don’t just write Falling hazard .

Write exactly what might happen to someone, including all the Ws. Who, when, where, why and how. (Okay, that last one isn’t a W.)

This shows you have fully understood the nature and depth of the risk.

3. Be realistic with your risk rating.

After you’ve identified your hazards, you need to rate them in terms of the intensity of the risk. Your examiner will be looking to see how you’ve come to your risk rating so make sure it is realistic.

The risk rating column is divided into three parts labelled L (likelihood of hazard occurring), C (intensity of consequence should the hazard occur) and R (overall risk rating).

Taking into account the existing risk control measures you previously mentioned, your rating should be calculated using the IOSH 5×5 risk rating calculator, which works by multiplying together your rating out of five for likelihood and consequence.

The examiner will use the information you have provided and determine their own risk rating. They will award you a mark if your rating is within a ±1 range of theirs.

Based on the existing controls and your risk rating, you’ll need to show you understand what additional controls to put in place and then be able to determine a new risk rating, following the same formula based on these proposed new controls.

4. Be specific in your suggested action guidelines.

The final two columns — Action/Monitored by whom? and Action/Monitored by when? — round off your risk assessment. To show the examiner that you have fully understood the process, you need to be very specific here.

Under Action/Monitored by whom? , you need to allocate a specific job to an individual with a specific job title, demonstrating that you know what action must be taken and who is best fit for the job.

Under Action/Monitored by when? , you need to fill in a specific date to show you have understood the level of urgency that the problem carries.

Consider your existing and suggested risk control measures when deciding on the action to be taken and account for the level of severity suggested by your risk rating when deciding how urgently action must be taken.

In order to receive an IOSH Managing Safely certificate, learners must complete the following:

  • The course's learning materials, which are split into seven modules.
  • A multiple-choice question paper.
  • A practical risk assessment project.

In this guide, we will look at the practical risk assessment project in detail and provide information on how to successfully complete one.

What is the IOSH Managing Safely practical risk assessment?

The IOSH Managing Safely risk assessment is a practical project that forms part of the IOSH Managing Safely course assessment. It requires you to complete a short risk assessment in a workplace of your choice, which should be your own where possible.

While a normal risk assessment will require you to identify as many hazards as possible, this project only requires you to identify four. These hazards will vary depending on the workplace, but may include using hazardous substances, or frequently carrying out tasks that involve manual handling.

For each of the four hazards, you need to list:

  • Who might be harmed by the hazard and how.
  • What the existing control measures are.
  • The current risk rating.
  • The additional controls that could be put in place.
  • How these controls change the risk rating.
  • Who will implement the additional controls and when they will do so.

We will look at how to complete the risk assessment project in more detail later.

How long does the IOSH Managing Safely practical risk assessment take?

Your risk assessment project must be submitted within 14 days of completing the course and the question paper. It should take you no longer than 2 hours to complete, but you are free to spend as long as you like on the project as long as it is returned within this time.

We recommend that you work through the assessment in one go, and then return to it a few days later to review it and make any necessary changes before submitting it to us.

What are the stages of the IOSH Managing Safely practical risk assessment?

Top of the form (1 mark)

This section is fairly self-explanatory. It requires you to fill in your name, the date and time of the assessment, and a brief description of the work area and the task being assessed.

When describing the work area and task being assessed, make sure you are specific when doing so. For example, third floor office space used by up to 30 people, 700 square metres , or use of forklift truck to move goods from loading bay to racking .

What is the hazard? (1 mark x 4)

The first column is where you identify the hazards associated with the task that are not being adequately controlled. Make sure that this hazard is significant (something with the potential to cause harm), otherwise you risk losing all of the marks for that row.

Also, make sure to outline the hazard and describe it in full. For example, carrying heavy boxes of books from racking to the shop floor would be an appropriate description, while Manual handling would not.

Who might people be harmed? (1 mark x 4)

List the full range of people that might be harmed and, where possible, quantify this list. For example:

  • The organisation’s maintenance team (2 people).
  • Up to 1500 members of the public each day.

When completing this column, be sure to think very carefully about who may come into contact a hazard. Is this hazard something that visitors, contractors or members of the public may be affected by too?

Existing risk control measures (1 mark x 4)

In this column, write down the existing risk control and safety measures that are in place to control the hazard. If you are struggling to think of any, be sure to consider ‘soft’ controls such as training, supervision, safe systems of work and inspections.

Risk rating (1 mark x 4)

To complete this section, decide on a likelihood rating (L) between 1-5 and a consequence rating (C) between 1-5, then multiply these together to obtain a risk rating (R) out of 25. Remember to be realistic with your ratings - you may be penalised if your judgement differs significantly from that of the examiner.

If you are struggling with this section, remember that it is covered in detail in module 2 of the course.

Additional controls (1 mark x 4)

Think about the controls you could put in place to reduce the risk and prevent ill health. For example: a wet floor, controlled only by signage, could be further controlled by installing anti-slip matting and placing a mop nearby so that any spillages can be cleaned up immediately.

New risk rating (1 mark x 4)

Use the same risk rating calculator as before to calculate a new risk rating out of 25. Make sure that your residual risk rating is lower than your original risk rating.

Action/monitored by whom? (1 mark x 4)

Assign a person/job title to each additional control. For example: provision of high visibility jackets to be completed by the facilities manager .

Action/monitored by when? (1 mark x 4)

Assign a date to each additional control. For example: new training course for staff that work at height to be completed by 15th December (two months) .

Bottom of the form (1 mark)

Put a sensible review date, which should be between 6 and 12 months from the date of the assessment, sign the assessment (an electronic signature is acceptable), and fill in the training provider name.

What is the pass mark of the IOSH Managing Safely practical risk assessment?

The pass mark is 23 out of a possible 38 marks, which is just over 60%.

The project is broken down into a number of parts. Please complete each section asdetailed below.

Part 1 - description of the work tasks that are your responsibility

Record a brief description of all the work tasks within your work environment including:

• description/labelled sketch of the location(s)• people who work in or visit the area on a regular basis or from time to time. (Think

about the different groups of people who may be in the area and how often theymay be there.)

• permanent and temporary items of equipment and substances• activities carried out within your work environment

Record a brief description of three location hazards that may arise in your workenvironment. For example, car park, stairs, uneven floor. For each of these hazardsplease include:

• a description of the location• a description of the hazard - in relation to the location• the number and occupation of people who could be affected by the hazard - in

relation to the location• whether a risk assessment is recommended

Part 3 - a hazard checklist of permanent and/or temporary items of equipment andsubstances used in your work environment

Part 4 - a hazard checklist of the activities carried out within your work environment

Record a brief description of three hazards that may arise from within your workenvironment. For each of these hazards please include:

Note: for the purpose of this exerc;:ise all three activity hazards that you areassessing on part 4 must require a risk assessment.

Part 5 - carrying out a risk assessment

Please carry out a risk assessment on all three of the activity hazards you have listed onpart 4. Please make sure you include:

• a description of the work activity• a description of the hazard, hazardous event and expected consequence - in

relation to the work activity• the number and occupation of people affected - in relation to the work activity• an assessment of risk (please use the 5 x 5 matrix) - in relation to the work activity

Note: for the purpose of this exercise all three activity hazards that you areassessing on part 5 must require risk controls.

Please complete the risk control form for all three activity hazards you have listed on part5. Please make sure you include:

• a description of the work activity and risk level from the risk assessment• existing risk controls• any further risk controls required - in relation to the work activity• the residual risk (please use the 5 x 5 matrix) - in relation to the work activity• a description of the type of monitoring required and how often - in relation to the

residual risk

Note: where you have large numbers of hazards in your work environment you will nothave space to record them all. In these circumstances, select a representative sample ofhazards for the purposes of this project and record only these. Remember that only threehazards are required for parts 2, 3, and 4.

Each part has a maximum number of marks which can be awarded and details are givenin the table below.
